Whitby Physio Centre Ltd. is committed to protecting your privacy. This statement explains how we handle your personal information, including personal health information, in keeping with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and the requirements of our health professions' regulatory colleges.

Information we collect

We collect the information we need to care for you and run our clinic. This may include:

• your name, date of birth, and contact details
• your health history, assessments, treatment plans, and progress notes
• insurance, benefits, and billing details, including WSIB or motor vehicle accident claim information
• information from other health care providers involved in your care, when appropriate

How we use your information

We use your information to assess and treat you, coordinate your care, book and manage appointments, bill you or your insurer, and meet our legal, professional, and quality-of-care obligations.

Consent and sharing

We rely on your consent to collect, use, and share your personal health information, unless the law allows or requires otherwise. Your consent may be implied when we share information with other providers for your health care. We ask for your express consent before sharing it for most other purposes, such as with a family member or an extended health insurer.

We may share information without your consent when the law permits or requires it, for example to meet WSIB reporting requirements, respond to a regulatory college or court order, or reduce a significant risk of serious harm.

You may withdraw or limit your consent at any time, including asking us not to share certain information with certain people. We will explain any effect this may have on your care. A withdrawal does not apply to anything already shared, or to sharing the law requires.

We do not sell your personal information. Service providers who support our clinic, such as software or billing providers, may handle information on our behalf and only as needed for that work.

How we protect and keep your information

We use physical, technical, and administrative safeguards to protect your information, and we limit access to those who need it. We keep records for as long as the law and our regulatory colleges require, then dispose of them securely.

If your personal health information is lost, stolen, or accessed without authorization, we will notify you as soon as reasonably possible and report to the Information and Privacy Commissioner of Ontario when required.

Your rights

You generally have the right to see and get a copy of your records, and to ask us to correct them if they are inaccurate or incomplete. Send your request to our privacy contact below. We will generally respond within 30 days, and we may charge a reasonable fee for copies. In limited cases the law allows us to refuse access, and we will explain why if so.

Phone, email, and WhatsApp

Email and messaging apps may not be fully secure, and messages pass through outside services. Please don't send medical details by email or WhatsApp. We'll discuss these at your visit or by phone.

Our website

This website does not ask you to create an account, and we do not use it to collect your health information. What you type into the chat assistant stays in your browser and is not sent to us. Like most websites, our hosting provider may keep basic technical logs. Our fonts and the map on our contact page are provided by Google, which may receive technical information such as your IP address and may set its own cookies when you view the map.
